Policy

According to the Florida Board of Governors and FIU policy, all international students with F and J visas statuses are required to have medical insurance before registration. Documentation must be submitted to the Health Compliance Office via email to insure@fiu.edu.

Students will receive an "IMI" insurance hold on their student account. Students will not be allowed to register for classes or continue enrollment without complying with the medical insurance requirement. Temporary hold removals will not be granted by our office. There are no exceptions to this rule.
   
F1/J1 visa student Summer class enrollment is optional, but insurance coverage is MANDATORY, regardless of overseas, in-person or online status.

For students with insurance coverage through their government’s embassy, the only acceptable document for compliance with insurance requirements is the FIU Alternative Insurance Compliance Form, which must be completed by an embassy representative for the current term(s).

If you are a continuing sponsored student, please be advised that you must submit a new FIU Alternative Insurance Compliance Form every semester to ensure you maintain continuous coverage throughout your time at FIU. New forms must also be submitted every time you extend your current policy or purchase a new insurance policy. Failure to comply with this request may result in termination of your SEVIS record. Holds are not removed without status updates.

FIU-sponsored health insurance plan

FIU partners with the State University System’s Insurance Broker, Gallagher Student Health & Special Risk, to provide international student health insurance coverage through UnitedHealthcare Insurance Company.

FIU UnitedHealthcare Insurance plans will only be available for purchase online via the Gallagher Student Health & Special Risk website. For your convenience, these plans are sold annually, or per semester, in accordance with the academic schedule.

Note: Online purchases have an e-check fee of $3.00 or a credit card processing fee of $10.00. Monthly payment options will not be accepted for any international students. Insurance coverage must be paid in FULL for the semester.

International Students should create an account on the Gallagher website by clicking Sign up or Log In under the Profile section. Please Note: If you had an account with Gallagher on the version of the website prior to Fall 2021, your previous username and password may not be accepted, and you will need to create a new account. Click on the link for 2021-2022 FIU Student Health Insurance Plan-International Students under Coverage Options and complete the enrollment form. After all form fields are completed, click “submit.” You will receive a confirmation email from Gallagher once the enrollment form has been submitted and another in 24 hours with a link to complete payment. If no link for payment is received, please contact Gallagher Customer Service at enrollmentteam@gallagherstudent.com. Once payment is processed, the student will receive a Confirmation of Payment email and receipt from the Gallagher website. The student must forward the receipt to insurance@fiu.edu to remove their insurance hold.

Alternative health insurance policy

If you have your own insurance policy, your insurance company must complete and send the International Student Alternative Health Insurance Compliance Form directly to FIU-Health Compliance Unit at insure@fiu.edu

Note: The FIU Health Compliance Unit will not be able to review any alternative health insurance plan without the Alternative Health Insurance Compliance Form. The guidelines for insurance have been established by the Florida Board of Governors and must be met. Please review the Florida Board of Governors state regulation before purchasing any alternative plan.

The completed compliance form must verify the following:

  • the coverage meets or exceeds the minimum FIU requirements
  • the name, address, and telephone number of a claims agent in the United States
  • you are covered under the policy
  • your insurance has been pre-paid through the current year

Also, the Health Compliance Unit must receive the International Student Alternative Health Insurance Compliance Form directly from your insurance company. Your policy will be reviewed, and if it complies with the guidelines, you will be cleared to register. If your policy does not comply with the guidelines, you will not be cleared until you provide documentation of insurance coverage that meets FIU requirements; or purchase the FIU Sponsored Health Insurance Plan.

FIU approved alternative insurance plans

International graduate assistants

Florida International University provides subsidized health insurance to all Graduate Assistants, Teaching Assistants, and Fellows holding a full-term appointment for 20 hours per week. All graduate assistants will be enrolled in the new Graduate Assistant health insurance program and will be cleared of the insurance hold (IMI) from a list provided by the Graduate School.

Graduate Assistants will pay a percentage of the insurance premium through payroll deduction. All Graduate Assistants are required to participate in this health insurance plan unless they wish to opt-out and provide proof of alternate insurance by submitting an alternate insurance form to Student Health Services and UGS.
