Psychiatry Appointment Scheduling

Scheduling an appointment with a behavioral health provider at the Student Health Clinic:

Call 305-348-8385
Inform patient access representative if you are currently seeing Dr. Carlos Sandoval or Dr. Baharak Khabazghazvini at the FIU Counseling and Psychological Services.
After you scheduled your appointment date and time, visit shs-portal.fiu.edu to complete the required forms and obtain your zoom link.

Go to shs-portal.fiu.edu
Login with your FIU username, which should be the first part of your email (e.g. if your email is roary001@fiu.edu then you would use roary001) and password.
Login through 2-Factor Authentication if prompted.

After a visit to the clinic, a summary of the visit will be posted on your account, as well as any related documents. To access these documents:

Login at shs-portal.fiu.edu
On the Appointments tab at the top, go to View Appointments.
Here you can see Upcoming and Previous appointments. To view the summary for a previous appointment, go to the right side of the appointment and select Actions, then select Encounter Summary. This will download a PDF of the summary.
