Employer, Family, and Other Plans NOT Pre-Approved

As you have chosen to purchase a health insurance policy that is not the FIU-sponsored plan or one of the pre-approved health insurance plans, please note that the Health Compliance Office will not be able to review any alternative health insurance plan without the completion of the FIU Health Compliance Insurance Checklist and the submission of all required documentation listed on the form.

You and your insurance company must complete and submit the following required documents:

The form must be fully completed; otherwise, it will not be accepted and will be returned for completion. The student may complete their section, but the remaining portion must be completed by the health insurance provider, broker, insurance company, employer representative, or carrier.

Certificate of Coverage (Policy Details)
An official document in English from your insurance company such as Summary of Benefits, outlining the terms, benefits, exclusions, and conditions of your plan. The certificate must be for the current year of the policy and cannot be from prior years.
Insurance Card (Front & Back)
A physical or digital insurance ID card issued by your insurance provider. The card must be active/valid and include the student’s name and coverage dates.
Proof of Pre-Payment
Monthly payment plans will not be accepted. All premiums must be paid in full for the entire coverage period, and proof and/or receipt of full payment is required.

The guidelines for insurance have been established by the Florida Board of Governors and must be met. Please review the Florida Board of Governors state regulation before purchasing any alternative plan. Health Compliance will not accept any plan that does not fully meet the state regulation.

Your policy will be reviewed, and if it complies with the guidelines, you will be cleared to register. If your policy does not comply with the guidelines, you will not be cleared until you provide documentation of insurance coverage that meets FIU and Florida Board of Governor’s requirements; or purchase the FIU Sponsored or FIU Alternative Pre Approved Health Insurance Plan.

Please be aware that there is a 48-72 hours processing time during weekdays for all incoming documents. To check the status of your hold, you may log on to your my.fiu.edu account.

Note: All requirements must be provided to our office for review. Failure to provide these requirements will result in denial of alternative insurance policy. Temporary hold removals or exceptions are not allowed under any circumstances as per FIU policy and Florida Board of Governors regulations for international students.
