FIU CAPS and VEP Records

CAPS and VEP maintains confidential records in accordance with state law. Records are maintained for 7 years past the last date on which the last service was provided. Information regarding contact with our center will not be disclosed to anyone outside of CAPS and VEP without the client’s written authorization to release the information except as mandated or allowed under Florida and/or Federal Law.

In situations where an exception to confidentiality protection may exist, CAPS and VEP will make an effort to discuss the circumstances and procedures for the disclosure with the client and enlist the client's assistance in resolution of the situation.

Requesting Records

  • Complete the online form. A representative from CAPS and VEP will contact you within 3-5 business days (the information you share will be transferred by automated email to CAPS and VEP professionals).
  • After you complete the required forms, CAPS and VEP will respond to your written request within 30 business days. You will be notified when the records are ready.
  • Responses to requests for information contained in a confidential record will be delivered in person, via fax, via USPS Certified Mail, DocuSign, or to FIU emails.
  • Bring a photo ID to pick up records in person.

Please Note: You may also speak with your regular clinician about records requests during a scheduled session. CAPS does NOT provide forensic psychological assessments and related documentation for:

  • Emotional support animals
  • Housing contracts/living alone
  • Workplace disability
  • Court-mandated treatment or evaluations

To make an appointment with us, please call 305-348-2277 (CAPS) for both MMC and BBC offices.

Appointments will be held in person or by videoconference (through a HIPAA-compliant platform).
