FIU CAPS/VEP Records

CAPS/VEP maintains confidential records in accordance with state law. Records are maintained for 7 years past the last date on which the last service was provided. Information regarding contact with our center will not be disclosed to anyone outside of CAPS/VEP without the client’s written authorization to release the information except as mandated or allowed under Florida and/or Federal Law.

In situations where an exception to confidentiality protection may exist, CAPS/VEP will make an effort to discuss the circumstances and procedures for the disclosure with the client and enlist the client's assistance in resolution of the situation.

Requesting records

Complete the online form. A representative from CAPS/VEP will contact you within 3-5 business days.
After you complete the required forms, CAPS/VEP will respond to your written request within 30 business days. You will be notified when the records are ready.
Responses to requests for information contained in a confidential record will be delivered in person, via fax, via USPS Certified Mail, or to FIU emails.
Bring a photo ID to pick up records in person.

Please note: you may also speak with your regular clinician about records requests during a scheduled session.

In addition:

CAPS does NOT provide forensic psychological assessments and related documentation for:

Emotional support animals
Housing contracts/living alone
Workplace disability
Court-mandated treatment or evaluations

Use this form if you need a clinician or a professional member of our staff to answer questions for a media interview.

Make sure to submit the form at least 2 weeks in advance (Please note that if you want to interview a clinician you must submit the form at least 3 weeks in advance). Requests that are placed after that time frame will be discarded due to high demand and scheduling purposes.

To make an appointment with us, please call the offices at MMC or BBC. Appointments will be held over the phone or by videoconference (through a HIPAA-compliant platform).
